Importing CSV/Excel into Sugar and creating fields

I am a newbie and am trying to create a commission system to run through sugar. I have a csv file and would like to import to the respective fields. i have attached the current csv (which will be tab delimited) file and would like to know how to create the fields in sugar so when i do the import the mapping will work correctly.

This should help you create your fields that you need:

Once you create the fields and add them to the layout, they will be available in the module's import tool.
