    Smile Fix for 5.0b Mail Merge Entire List

    You will find that when trying to mail merge an entire list in 5.0b you receive a php fatal error.
    This is due to the pearDatabase instance not being declared in the in modules/MailMerge/step1.php under the entire list condition.

    Attached is a replacement for step1.php with the needed code fixes.
