Can sugar be easily configured to handle client mamangement in a charity context helping people on the phone and generally recording a case (case management?) history? Has anyone done this?

IMO, for maintaining clients/customers SugarCRM is the best choice. It is suitable for charity too.
It would be useful if you would define your requirement in more detail. SugarCRM is very capable of client management/recording of calls/meetings/cases etc.
And if this is all you require your CRM to do then I have no hesitation in recommending Community Edition .... but do you need Sugar to do anything else?

as the others mentioned above, Sugar can definitely do this. Sugar can work for any industry, and can be customized to your needs. There is an out of the box Cases module, but salesagility is correct, we would need to know more details to best determine what your needs are and how best to use/customize the system to your needs.
