Create Lead Form - email field absent?

I know the documentation for 4.5.1 is not yet available and perhaps that will explain my problem, but I was anxious to try the new lead form builder functionality. I have had two problems: First, all the information on email is skipped completely by the form builder for some reason. Second, it didn't work when I tried it, though it went through the actions and provided my chosen response page. If anyone has used this feature and has some advice I would appreciate it. I will get into it further this weekend and see if I can find the culprit.

I watched the new video tutorial on the form creation tool and it looks like it though she chooses the email field, it does not get displayed in her resultant form either. Is there a reason this is suppose to take place or am I experiencing a problem?

I'm having the same problem with generating the email field in the web-to-lead form. If anybody has a fix, please post the code. Thanx!

Fixed it. In the HTML source, edit as follows:
