Custom fields added in Sugar 4.0

We are new to SugarCRM and switching from ACT. Two questions for your old pros...

1) Are new custom fields added to SugarCRM specific to the specific module only or are they system wide?
2) If they are specific to a given module, how can you have values entered in the custom fields pass to another module? This is specifically related to custom fields in 'Leads' that we want passed through to the 'Account' when the Lead is converted through the SugarCRM 'Convert Lead' process.

If you use Sugar Studio - the custom fields created are specific to the module, and you will have issues passing them to other modules. I believe you will need to have custom software development to address this.

I assume when you mnention custom software development, the field 'passing' could be modified in the SugarCRM source code by our technical staff? Thanks...
