How do we modify an On Demand - Hosted instance?

We are a customer of the Sugar On Demand Hosted version (Professional). A very basic question, but it seems to elude all the documentation and marketing-speak:

Can a Hosted customer modify their instance on the Sugar servers? If so, how? If not, does that preclude using any Sugar Exchange modules?

Also, what is the timings for the upgrade path from version 4.5.1 h to 5.0 beta if you are a hosted customer?

All the documentation I've read doesn't distinguish between Sugar installations and Sugar hosted versions.
