Issue in Create Tasks under Contacts Subpanel

I had an issue explained below:

Click "Create Task" from the Activities Subpanel
Issue #1: click on the Contact Name Text box, then click some other label in the form. Contact Name Textbox field get cleared.
Issue #2: Select anyitem from the Related To dropdown and you can see the values next to this field get cleared. This is not an issue. Then click on cancel. Again click on "Create Task" and select anyitem from the Related To Dropdown, you can see the field next to this drop down get disappeared.

Hi,

I have no solution - but I noticed a very similar issue just now. Also, an apparently related issue where a select box can pick multiple records (!) when you tab/click into and out of it.
