Subpanel Fatal Error

Using Sugar Open Source
Version 4.2.1b
$sugar_config['hide_subpanels'] = false;
$sugar_config['hide_subpanels_on_login'] = true;

When I am in Account view and
1) have the activities, history and contacts subpanels showing
2) click create note
3) click cancel

I get the following error:
Fatal error: asubpanel() [function.require]: Failed opening required 'modules//subpanels/.php' (include_path='.:/usr/local/lib/php') in /homepages/28/d160214145/htdocs/SugarCRM/include/SubPanel/SubPanelDefinitions.php on line 67
