Some caveats:
1) The Access tables do not match the Sugar Mysql tables, so you would need to modify all queries.
2) We are not yet convinced this can be done (we tried getafreelancer.com but all we got back was "we do PHP" and no-one seemed to really understand the scope of the problem.)
3) Less than 5 users at any one time, so record locking issues (between sugar and access) can be ignored.
4) Again, altering the Access tables in any way is impossible because an existing app uses the tables.
