4.5.0d upgrade - page displays all wrong

The layout after upgrading from 4.2.1a to 4.5.0d is all wrong, with the left panel of shortcuts taking up most of the entire page, and the dashlets off to the far right. I also get a bunch of javascript errors just trying to load the Home page:

Noted that this is some sort of bug in the upgrade of the Sugar theme. We find that the other themes render fine, so we're using Sugar Classic for now.

Is there a "Sugar" theme patch that we can apply to get it back to working order?

Is there a way to get the Sugar theme code installed on top of my 4.5.0d release that fixes our layout issues? It seems hard to believe everyone's Sugar theme is broken as that must be the default for testing.

However, we cannot use Sugar theme because the "small left side panel" expands to fill most of the page, and main text can only be seen by scrolling to the far right. Sugar Classic theme works fine, so it appears to be related only to themes and not anything else with the software.
