4.5.0d Upgrade wizard error: Table 'sugarcrm.emails__uw_temp' doesn't exist

Hello,

Upgrade wizard error during upgrade from 4.2.1c to 4.5.0d
The error message was:

Errors Found During Preflight Check: Show Bad Queries Table 'sugarcrm.emails__uw_temp' doesn't exist:: ALTER TABLE `emails` ADD COLUMN `raw_source` text AFTER `mailbox_id`

Windows 2000 5.00.2195 (SP4)
SugarCRM version: 4.2.1c (patched)
Patch version: SugarSuite-Upgrade-4.2.1-to-4.5.0d.zip
Apache 2.0.52
PHP 5.1.2
MySQL 4.1.21-community-nt via TCP/IP


Any help/hacks will be greatly appreciated.
