5.0 create account does not work from search popup

We're using version 5.0.0g

When creating a new record or editing existing record, for instance in Contacts, when we click on the Search button to bring up the Accounts lookup in order to associate an account with the contact, if we then click the Create Account button in the search popup window and input some data (account name, etc) and click Save button, the new account is not created. This fails in every module.

It is not due to an ACL rule, I'm an admin.

Any fixes out there?

p.s. I'm happy to hack the code if needed.

Just installed 5.0.0h patch, did not fix the problem. Nervous to upgrade to 5.1, as this is a production system.
