6.1: no results in targetlist sort by user, "base implementa", $account_name !replace

We installed 6.1.6 a few weeks ago when the RCs were out and having menu issues. Since then, we've found a few bugs. This is a completely stock installation with no plugins installed. I was hoping I could find out where/how to fix them.

Sorting by user in the target list finds no results:
Click on Site Map > View Target Lists. Then click on User (sort) on the right end of the table headers. As long as we don't sort by user, we have tons of entries in the list. By clicking on sort by user, the view doesn't show anything. Does anyone know where to fix this?

Breadcrumbs/History constantly shows "Base Implementa"
After browsing through some pages, the top menu bar shows "Base Implementa" repeatedly. The most I've found for this is that certain modules don't have certain functions defined to output the module title. But this is a stock installation. We didn't add any plugins. Clicking on the link gives us the following message "Error: File [modules/CampaignLog/metadata/detailviewdefs.php] is missing. Unable to create because no corresponding HTML file was found." Do I have to go make detailviewdefs.php outputting the string function for every module Sugar is doing this with? I would think someone has already done it because again, this is the stock install. Or is there something I'm missing here.

Email Editor has incorrect variables
We found that the variable inserter in the email editor references $account_name which doesn't get replaced. I changed the email view module to output $contact_account_name instead and it works fine. I figured I'd just let everyone know about it.

- This appears to be a bug in 6.1.3 as well but works normally for 6.2.
