Activities post in triplicate or not at all

OS:CentOS 5.2
SugarCM: Version 5.1.0c (Build 4979)
WebServer: httpd-2.2.3-11.el5_2
PHP: php-5.2.8-1.el5
MySQL: mysql-server-5.1.30-1.el5

I recently migrated from 4.x to 5.1.0c and I am noticing now that activities that are being created either do not post or are posted in triplicate.

Anyone know why this might be occurring?

Have you confirmed that the records in question are actually triplicated in the database, as opposed to it being a display issue?

How would I even begin to look for that?

- phpmyadmin shows 112 tables for the sugarcrm database.
- All the data is in jiberish when I browse it in phpmyadmin.
- All links the have the same subject have three differnet links that all go to the same location:
1. sugarcm/index.php?module=Calls&offset=12&stamp=1236116176022665900&return_module=Calls&act ion=DetailView&record=f38a05f2-847d-d4b5-7db7-473de45eaefc
2. sugarcrm/index.php?module=Calls&offset=13&stamp=1236116176022665900&return_module=Calls&act ion=DetailView&record=f38a05f2-847d-d4b5-7db7-473de45eaefc
3. sugarcrm/index.php?module=Calls&offset=14&stamp=1236116176022665900&return_module=Calls&act ion=DetailView&record=f38a05f2-847d-d4b5-7db7-473de45eaefc

the offsets are different but that is just because of where they fall in the results.

I dumped the sql database and looked at the tracker database and noticed lots of triplicate entries. What php file writes to that table?
