Add Contact Fails - Shows Screen With No CSS

This does not occur for all logins...but its reproducible for one user every time.

From the account screen, I have a user who attempts to add a contact under the Contacts module. Upon doing so, he is displayed an unformatted white screen with an unrelated Contact's name pre-filled in the search field. I dumped the entire MySQL database and cannot find the user "Ned Wilson" in the database. Can anyone give me some direction around where this contact entry is coming from? It seems like it may be an IMAP sync issue or something external.

Have you tried resetting the user's preferences in the Admin?
