Campaign name shows up in email subject

When we send out emails via a campaign Sugar has been adding the campaign name before the subject line.

e.g.
To:
From:
Subject: [Campaign name]: [email subject line]

How can we make it not add the campaign name?

ETA: It seems to be erratic -- sometimes it adds the campaign name, sometimes it doesn't.
