We used just the letters cn for the bind attribute and the login attribute is samaccountname
The base dn for us is: cn=users;dc=insightsnow;dc=com
It's been so long since I've done this I can really only give you the examples we have set. Also, you can have redundant LDAP servers provided you use IP's. Just separate them with commas.

Here in the forum and in the WIKI a lot of tipps have been published in the last year concerning LDAP and its settings.
It is important which LDAP you use, without any information about your system it could be difficult.

BIN attribute: ?????. HERE TO INCOME
login attribute: sAMAccountName
authenticated user: adminstrador@servidor.com (is active directory)
........ password user Active Directory
does not allow the connection of the user,
ldapauth: failed LDAP bind (login) by xxxxx, could not construct bind_user
