Could not connect to server localhost as sugarcrm.Too many connections

Any thoughts about what could cause this error: Could not connect to server localhost as sugarcrm.Too many connections

It has happened with HTTP connections from SOAP and a browser. I'm on 4.5 with Apache2.2 and PHP5.

It appears that PHP is leaving the database connections open. I have no experience with PHP -- does it normally leave connections hanging around? Is this a PHP config type thing?
