Create Account not working in Select Account Box

After creating a new contact, I hit the "select" box next to Account. In that box I hit "Create" and fill in the fields that pop up below the button. When hit "save" to save the new account, it is not created, and I am returned to the "select account" box.

Check your sugarcrm.log file for error messages. if none, set your logging level to 'debug' in the log4php_properties (or something like that) file in the root directory of sugar, then try again and check for errors.
