Create Additional TextField

I am trying to create a additional textfield that say "Account Name" and a input box. See Picture Below.



what php files do I do the editing?

Basically I am try to replicate the search function of Contacts module. What I need is a input field and the buttons next to it. I can't find where the codes are that Contacts module used to create the Account Name input field.

This is for another module, so I need create this.
