Custom Module Config Not Saving

This is the situation. I had some data that needed to be imported with a number of custom fields. I created those custom fields in the studio, uploaded all the information into the contacts, and everything worked well. Now, when I go to create a contact my custom formatting and fields don't show up. Instead I see the default format for inserting a contact. So I go back into the studio and under the contacts module it shows my custom setup still. So I proceed to save and publish, and it makes changes to the contacts already in the database, but when I go to add a new contact - once again I see the default format. There are a number of other adminstrative users that have been on the account, but even in all of those other accounts the studio still shows the custom format, but when I add a contact I still see the default setup.

Here are my system specs:
SugarCRM 4.5.1a (newest version just updated)

I am not positive about the server information, but I did a dianostic and I can get any needed information.

I want to be able to save this custom contact format obviously, and at one point when I went to add a new contact it showed the custom format. After a few days it reset back to the default and I cannot get it to return to the custom.

Any suggestions?
