Hi vishwas
Is your system installed in Linux or Windows?
Check the file permission for folder cache.
Also check the max_upload_size in the php.ini

I have had the same problem in my own Sugar instance so I tested my actions with a demo version on the Sugar site (CE). The same issue happens. I can create the module and see the subpanels as defined by relationships, but there is no tab on the top menu. I check "Configure Tabs" and the new module does not appear on the list (display or hide tabs). When I create an item from the module subpanel and then edit it I am taken to the custom module "tab" which appears as a "blank" tab on the menu. Am I missing a step along the way?
