document revision

HI,
I am using SugarmCRM4.5.0h.I have upload document via soap using c#.net.There are five input parameters passing in document revision.The five parameters are filename,file(encode),document name.revision and id.The documents are uploaded successfully.But the document name is not display in the document list field.
Please help me.

try check the upload file related database table.
if the file name didn't save check the sugar log file.
