Our system keeps tanking on us a few days into starting to use it in production. We start adding in cases and everything is fine. We can even close the first few cases but then something is happening in the system and we can not close anymore cases, new or existing.

When ever we change the status to closed, we get the following error:
MySQL error 1062: Duplicate entry 'bc665977-8d2c-a66c-c34a-49f73bffc67b' for key 1.

We can change the status to anything else without a problem except closed.

We have blown away and reinstalled 5.2.0c 4 times now and don't know what the issue is.

We have only made two customizations:
1. We installed Process Manger 5.0
2. We added a status of "In Progress" to the project_status_dom drop down.
