Error In Upgrade Wizard

While in the Upgrade Wizard I get the following errors:

Notice: Undefined index: setup_db_type in /var/www/html/sugarcrm/include/utils.php on line 2283

Notice: Undefined index: setup_db_type in /var/www/html/sugarcrm/include/utils.php on line 2292

Notice: Undefined index: setup_db_type in /var/www/html/sugarcrm/include/utils.php on line 2292

Any idea what this means and how to fix it?

Additionally, when in the Upgrade Wizard it shows that the BugarCRM 4.5.1a patch is installed. But when I go to the "About" button, it show version 4.5.1 (Build 1197). Does this mean that 4.5.1a is or is not loaded? In any case How do I fix it?

Some additional information: These are the steps I followed:

1) Entered Update Wizard.

2) Clicked on "Next"

3) Got the following errors:

3) It said System Checks In Progress with the following on the right:

FIle Issues:
File Permissions All Files Writable

Database Issues:
Database Privileges All Privileges Available

Server Settings Check:
Detected Settings All system settings requirements satisfied

4) I clicked "Next" and the "Upload Upgrade" said In Progress and a message appeared that stated. Required steps must be completed before continuing. While on the "Install from Sugar Exchange" tab SugarCRM searched but did not find any updates.

5) Clicked on "Install from local file" and selected the SugarCRM 4.5.1a update file.

6) Next I pressed "Upload Upgrade".

7) The program returned to the "Install from Sugar Exchange" screen and displayed this message:

Required steps must be completed before continuing.
SugarOS-Patch-4.5.1a.zip has been uploaded.

8) At the bottome the 4.5.1a is listed as instaled, not ready to be installed:

The following upgrades have been installed:

Name Type Version Date Installed Description Action
SugarOS patch 4.5.1a 2007-03-02 07:42pm None

9) When I go to "About" the version is reported as: Version 4.5.1 (Build 1197)

Does anybody have any ideas as to what happened and how I can fix the above reported errors and get the 4.5.1a update installed?
