Hello , I have a problem with attaching a file to a account, I go to the detailview of the account and then to the subpanel History , and add a note, I click full form and then when I attach the file and save i get this error:

Fatal error: TeamFormBase::require_once() [function.require]: Failed opening required '' (include_path='.;C:\php5\pear') in C:\Inetpub\wwwroot\Sugar\modules\TeamsOS\TeamFormB ase.php on line 65

If I do the tipical way it works:
-Click Activities tab
-Click "Create Note or Attachment"
-Select the account
-Choose the file
-And save

Any ideas? Thanks and excuses for my english.
