» Webserver type and version - Apache 2.2.x
    » PHP version - 5.2.1

    The simpliest approach is to
    1. backup sugarcrm folder and database
    2. install a clean 4.5.1d version of sugarcrm on the new server using same credentials and paths
    3. shutdown apache
    4. restore database on a new machine
    5. overwrite sugarcrm files on the new machine. You may need to tweak permissions.
    6. start apache

Make sure all the paths are the same as on the old server to make the process easier.

In future you may want to look into virtualization which allows hardware upgrades to be very easy.
