Help with Fatal error

Hi, I have this error:

Warning: require_once(modules/MySettings/Forms.php) [function.require-once]: failed to open stream: No such file or directory in /var/www/themes/Sugar/header.php on line 802

Fatal error: require_once() [function.require]: Failed opening required 'modules/MySettings/Forms.php' (include_path='/var/www/include/..:.:/usr/share/php:/usr/share/pear') in /var/www/themes/Sugar/header.php on line 802

when tryng to save a task or opportunity. Community Edition 5.2.0c
I have checked and the file Forms.php does not exist.

Any suggestion?

Did you upgrade from a previous version?

Yes my first installation was 5.2, then I have upgraded to 5.2.0a and 5.2.0c. Since I'm using only official repository in my debian linux, I cannot upgrade to 5.2.0f (php version problem).

Try repair home page and repair default preferences and try again.
