Help Making Forms

We have departments that currently use Access databases to capture information and do reporting. I want to "fold" this data collection into SugarCRM by adding user input forms.

Are there instructions for form creation?
How would I simulate this example:

(The image expires after 7 days)

Hi, umayxa3

SugarCRM has no instruction for creating forms, but if you have these data into a Access DB and the data stored into this one refers to modules in SugarCRM, like Accounts, Contacts, Cases, you could create an interface using the SugarCRM Soap API (soap.php) for inseritng records into SugarCRM database.
