History - Attachment & Note not saving

Thanks for the help.

I am the admin on our system - a user created an account that I need to add a History Note & Attachment (6MB PDF). I've tried numerous times - I create the note - it says saving, then saved. However, the note does not show up. I looked directly at the user's account - same thing.

Any ideas?

check your php.ini for max_upload size
