Home page resets periodically for users

I'm getting reports of the home page being reset occasionally so that customized dashlets and dashlet layouts along with Jotpad text gets reset to the original factory presets. Has anyone else come across this issue?

Sugar: 4.5.1e OS
Apache: 2.2
PHP: 5.2.2

This problem was due to database connections failing in our multi-db pool setup. I had to set autofree to true and disabled_persistent_connections = true to get this issue to go away.
