Default How do I recreate the billing address form?

I was redoing my Account Edit layout and somehow managed to move all the individual fields from the billing address to the Toolbox. I have been able to move them all back to the layout but they are individual fields and not the orderly form I had before. The form still exists in the Detail Layout.

How can I restore the form?

    Thanks

    admin>studio>accounts>layout>editview>restore defaults

    or

admin>studio>accounts>layout>editview>view history - then do a point in time restore

Thank you. I'l probably have to restore defaults. I tried the history option but must not have created a rollback position.

I do not see a "restore defaults" option on the screen and the view history does not store the original layout. Was this option dropped?

    Thanks
