How to install patches?

I've tried to load the patch 4.5.0f with some difficulty. The merging files was an issue. Then the browser screen went all white. (said "Done" in the bottom left corner). When I logged in again, I had a task about manual merge, and the admin screen indicated I was on version 4.5.0f. I didn't trust it so I restored my backup DB and sugar directory. I'm going to try again next week.

Questions:
I'm running 4.5.0d. Do I have to install both 4.5.0e and 4.5.0f patches?
When I run the upgrade wizard, and i get to the step about merging files, what should I choose?
Any other basics I should know about installing patches?

As long as you have not made customizations to the layouts or added any custom fields you are safe to overwrite during the upgrade. To force the overwrite during the upgrade or patch you show what is to manually merged and put a check in the box to overwrite all of them.

Sooo ummm Where is the official documentation on patching.... I've looked... I've searched.... !? Another little thought... if a date is required for authentication of the license... shouldn't you give me one!?
