How to upgrade from 4.5.0a to 4.5.0d?

I now get a message when I login to sugar saying I need to upgrade to 4.5.0d, but when I go to the sugar crm download page, I do not see a patch to go from 4.5.0a to 4.5.0d. How are people supposed to upgrade that are in these interrmediate versions?

You just apply the patch for 4.5.0d, it provides and update for all previous 4.5.0x releases.

I assume the upgrade wizard is to be used for applying patches? When attempting to use the upgrade wizard I get an internal server error.

Are there specific settings I need in pho.ini or other files to allow the upgrade wizard to work?
