It appears from the number of threads that this upgrade is fraught with problems/bugs. Excuse my ignorance but is there a complete idiots guide anywhere that walks me through this unscathed?

I have upgraded from 4.2.1 > 4.5a without problems. As with any upgrade you should not attempt this on a production server. I found it a fairly simple matter of setting up a new mysql database, copying all the files across and inserting the sql data using phpMyAdmin. The I tested the upgrade and once satisfied, simply switched over to the new server.
There is an awful lot of code that has gone into this new upgrade and this method of upgrading is the same with any new piece of software.

Thanks Chris so basically you did a completely new install with the latest version and then copied the data across. Sounds good to me as long as the database structures are the same. Are they?

No, I read somewhere (I think the release notes) that you must use the upgrade as some of the tables are adjusted etc. I agree it is fraught with problems, but the advice is sound, do the upgrade on a copy of your data and then if you want to you can copy that upgraded SQL data to a new install. We run all our CRM stuff on a virtual machine which is perfect (even for production) for this type of web app.
