insert form data into existing contact info --- help

Hello,

I have a form that has additional fields/data that i want to have populate into an existing contact(s). I created the additional fields for the 'Contact' page and would like to have the people who already have a contact entry to be able to fill out a new form that i send via email and when they fill out the form and submit, it will automatically insert the new data into the existing contact's info in the contacts section of the CRM.

Also, I'll be using an email template from the CRM which will include the contacts name ($full_name) and address ($ variables - if available in their existing contact) and will be using the CRM to send out the email template forms using select contact list.

Any ideas on how I can implement this? Any help would be greatly appreciated. Thank you!
