Issue with web to lead form

I have created a web form in the campaigns section of sugarcrm and inserted that form on a html web page. When i go in to the website and try to submit it says error on page. Im not sure why it does that. i copied the code from Sugar and pasted it on the web. Didnt change anything.

Please advise.

What version of SugarCRM are you using? I think that there is a bug in the 6.2.0 version that I am tracking down, it keeps the javascript submit_form function from running properly.
