Lead Capture form send to Accounts or Contacts

I have an extensive form that I need the results of to go into sugarcrm.

First, I need sugarcrm to be setup so that there can be 3 or 4 kinds of contacts/leads a) conference registrations b) leads c) current customers etc. I need a couple of different forms that can capture info and add it to sugarcrm as contact or accounts. I have added some custom fields but don't see where they are. One of the forms that I need is http://selectivemutismcenter.org/appt/Followup.htm

Can someone tell me the best way to handle this and if it can be done? Thanks.

It sounds like you are going to need to use the SOAP API to insert your data, as opposed to lead capture.

The SOAP API will give you much more flexibility in where data ends up in Sugar.
