Have you checked the DetailView layout via Studio to ensure that the address fields are still on the layout?

In the leads module editview they *do not* appear? (I think that's what you mean, so I'll answer that)
The issue seems to be related to your editviews defs. I am going to go ahead and assume that you've made customizations. if you haven't, then I'd need to see more than what you described (screenshots would be good)
Anyhow, There's probably some bad edits and you may want to start again. Try changing the directory name to back it up and logically remove it from sugar's path.
So like ./custom/modules/Leads/metadata/ to ./custom/modules/Leads/metadata.bak/
That will remove all your custom layouts (make sure you turn on developer mode Admin >> System Settings >> Developer Mode) and that should clear the server-side cache.
Hopefully you'll be able to fix the changes in studio again. And your search defs and such will be in that metadata.bak dir. YOu can save yourself the hassle of redoing the layouts by recovering the files from there.
When you're done, make sure you turn developer mode off - that will give you a performance boost.

ANGEL - The Address Fields are present in the Detail View. (as you can see from the screen shots)
MATTHEW - Yes you're right I meant they do not appear in Detail View but do appear in Edit View. I haven't made any customizations other than adding CE Teams which is supposed to be upgrade safe and doesn't seem to have anything to do with address fields.

You can see the fields exist for each but the data only populates in the Edit View and not the Detail View.
As I think about this, this is what I think is wrong...
The address is created/imported into various fields
Address Field
City Field
State Field
Postal Code Field
This is what is being shown in Edit view
It then should combine the above fields into one address field "Shipping Address" in the detail view.
For some reason the above data isn't populating into the "Shipping Address" field.
I don't know why and don't know how to fix.
