loading new module failed

I am using the on-demand crm and I have this error when I try to load a module that I had created
"Failed to copy /mnt/od2/store5/sugarondemand.com/ptn/cache/upload/upgrades/temp/GXR5uc/icons custom/themes"

any ideas?

I am also having this issue trying to create a new mod and load it. Any answers? it worked in 6.1 but hasnt since 6.2.3 upgrade running centos, mysql
