Do manual changes to source code of Sugar CRM create problems when performing an update in the future?

If so, what are these problems and can they be avoided or mitigated?

Or do manual changes just get lost without any additional problems?

This depends a bit on what you want to do. Many changes can be done upgrade save if the developer does it in the right way, in other cases the changes can't be done upgrade save.
