Multiple "Create Appointment" fields in Convert Leads

Hello,

I am running Sugar Suite Pro v4.5.1a, and was wondering if anyone could tell me how to successfully add an additional "Create Appointment" checkbox in the Convert Leads screen? An example of what I am trying to achieve for example is to have a "create buyer appointment" and "create seller appointment" fields.

I have tried to alter the convertLead.php file located in the modules/leads directory, and been able to have my screen display 2 instances of this checkbox, however the functionality of all the dropdown info that occurs when you check the box, does not work correctly.

Does anyone know the appropriate way to go about doing this so that Sugar will play along nicely with the changes, and accurately display and record the data for 2 of these fields?
