Look at the table that is used to link the entites (contacts, accounts = accounts_contacts ...) to verify that you have only one time your ID whith deleted=0

I'm having a similar issue using version 4.5.0h open source stack. In the list view for several modules it lists multiple entries with the same data. I'll use an example of an opportunity. At first our users thought they were duplicates and tried to merge them, but it returned an error "Please select at least 2 records to proceed". After troubleshooting I found that there was only 1 opportunity in the database with that name. I also found that the url for the link to the detail view, seen by mousing over the name of the opportunity in list view, was the same for each of the "duplicates". I confirmed that these to list view entries were the same opportunity because when I edit one, the changes are reflected in the other.
We've also seen this happen in leads and contacts modules with up two 3 list view entries pointing to the same database row. I've had one of our developers look at the source code and he reported back that he did not see anything that would cause this behavior. This is a big issue though, because we've already had 2 users try to delete one of the "duplicate" records, and it is kind of a pain to undelete an item and keep all of it's previous associations w/ notes, emails, etc. intact. (trust me its not as easy as setting the deleted flag to 0 for the item that was inadvertently deleted)

Actually, I should have read the thread more closely. SamuelAugy was correct, in the case of the opportunity, there were 2 accounts related to that opportunity. When I merged the two accounts, the second entry for the opportunity in the list view disappeared.
This does bring up a question, does anybody know the intended relationship types for modules? Because, it seems that the intended relationship for accounts to opportunities is 1:1, but it allows you to do many:1.
