pb with sugarsearch

Hi,
I try to use sugarsearch with the module http://www.sugarforge.org/frs/?group_id=414

In my candidate module, i have 3 new files :
searchform.html, vardef.php and searchfield.php in metadata directory (See attached)

All is bugged.
1- the "advanced research" panel doesn't appear
2- How to use sugarsearch with custom fields ?


any help would be very appreciated.
