Is it possible to change a Document you've uploaded

I have a module that I upload a document with. Can I edit the record and re-attach an updated document or do I have to create a new record each time?

In the standard Documents module, you can create "revisions" of documents and can upload the new revision, preserving the history of the previous revisions.

But for a custom module that I use to I will have to create a new record each time I update the document.

Is it impossible to do if you have something like 20 field, and 1 upload that change 1 time a day ...
