Problem importing csv file

Hi,

I'm trying to import csv file to Opportunities and got this message:

I did it many times in the past the same way and after upgrading the sugar the problem occur.

I'm using Version 4.5.1d (Build 1273)

I am also getting the same problem. Although I created a custom template and mapped all the fields including the required fields the problems in importing Opportunities persists. If someone has any idea how to resolve this, I would greatly appreciate it.
