Redirection Problem (Custome Module)

I have created a new module called Sample Request. when i save any request it is giving me this error after saving: (Error retrieving record. This record may be deleted or you may not be authorized to view it.), but when i check list of Sample Request record exists in the List. Please help me out

This is an access rights issue. Go into the admin panel and first of all rebuild roles through repair. Then check you have assigned access rights to the use you are using to access the record.
