Related Items Duplicating in Subpanel

I have a custom module that is related to my accounts module for some reason there are certain accounts where every entity in my custom module is displayed twice in the subpanel.
For instance, in the database, 51 objects come up as related to a specific account. The subpanel shows 102 objects, with each one obviously duplicated. The duplicate links both return the same object and work completely normally (barring the fact they're duplicated).
Repairing relationships does not solve this problem.

Any ideas on where to start looking?

Custom module 'my_custommodule' has many-to-many relationship with standard 'documents' module. The relationship was created in studio. SugarCRM creates duplicating records in 'my_custommodule_documents' table with the same my_custommodule_ida and documents_idb fields values for certain 'my_custommodule' records.
Documents were created from the docuemnts subpanel displayed on the my_custommodule detailed view.

Workaround solution is to delete duplicating records in 'my_custommodule_documents' table, but it's not so nice...

Yea... I've deleted the extra relations in the database and it's all back to normal, but I really would like to know what is duplicating them.

I've traced it down to the edit button in the subpanel.
Clicking on the item, then going to the edit menu is harmless.
Clicking the edit button in the subpanel duplicates the relation.

For now I have removed the edit button from the subpanel, but I am looking for an actual fix.
