Search no longer works after upgrade

I upgraded from Sugar Pro 4.2.0d to 4.2.1 and then to 4.5.0h and now the search as well as the clear no longer work.

The clear is returning undefined as the action and the module.

Any ideas on how to fix this?

I looked at the files to be manually merged but I don't see why it would affect the "Clear" button.
