Show fields if checked

Here is what I would like to accomplish.....not sure if it can happen.

Making a new module called PROGRAMS, at the top I would like to have 2 check boxes 1. Id theft 2. Credit Sweep. I would like to have all of the fields hidden unless one or both of the boxes are checked.

For example, if 2. Credit Sweep is checked I would like to display 5 other custom fields....otherwise hide them.....much like when you convert a lead, if you check Accounts, it will give you some fields to enter.
